Care management

Multidimensional dashboard of indicators for the healthcare management agreement.

Definición

Design a multidimensional dashboard to support healthcare management and decision-making in care pathways. Use automated processes to visualise key indicators and analyse data extracted from different sources within the Valencian healthcare system. This will improve quality, efficiency, and health outcomes.

The ultimate goal is to monitor results and analyse their evolution to determine whether objectives are being met and why. This will provide professionals with information to support decision-making, thereby improving the quality and efficiency of care processes and consequently positively impacting patient health and care.

Origin of the use case

Accurate and timely information is essential for modern healthcare management to assess achievement of objectives, optimise resources and ensure quality care. In practice, however, data is often fragmented across different systems and levels of care, which makes it difficult to obtain a comprehensive, up-to-date overview of the healthcare system.

Management indicators were conceived as a key tool for measuring results and guiding institutional strategies. However, their potential was limited by the lack of integration between information sources.

The Management use case was developed in response to this issue. It allows data from multiple sources to be consolidated into a single environment where it is transformed into useful knowledge that supports evidence-based decision-making.

The Integra Cloud
solution

  • Integra Cloud was created to respond to one of the greatest challenges in healthcare management: transforming large volumes of scattered data into useful, coherent and accessible information for decision-makers.
  • The system automates the extraction, transformation and analysis of data from different sources within the Valencian healthcare system. Thanks to this integration, it is possible to obtain up-to-date and reliable information on the different levels of care.
  • The dashboard allows for data exploration and trend detection, turning data into a tool to support improvements in efficiency, quality of care and health outcomes.

Results

The dashboard has established itself as an essential tool for clinical and strategic management. It is expected to contribute to improving coordination between levels of care and reinforcing a culture of data-driven decision-making.

Among the most notable results of this use case are:

  • Improved accessibility and efficiency in primary care.
  • Development of specific strategies for managing complex chronic patients.
  • Promotion of the use of tools to identify complex chronic patients who could benefit from integrated care.
  • Support for the planning and monitoring of institutional objectives.

This approach not only improves patients’ quality of life, but also allows for better management of healthcare resources, avoiding unnecessary interventions and repeated hospitalisations.
